What We Do

When authorized by the team, PACE provides all needed preventive, primary health, acute and long term care services so that older individuals may live in their own homes and communities as long as possible. We believe that older adults with chronic care needs do better when they are able to remain in their homes and in their community.

  • Other specialty services and equipment when needed

Many of these services are provided at the PACE adult day health center. Participants receive full medical and preventive care, and may participate in the range of activities offered each day. Care is provided by a team of licensed professionals who work to meet the individual needs of each participant. The team remains involved in the participant’s care even if they should need to be hospitalized or if they require short or long term treatment in a nursing home. The care team is made up of primary care physicians, nurse practitioners, registered nurses, social workers, therapists, personal care workers, and other dedicated staff. A PACE Participant must receive all needed health care, including primary care and specialty physician services (other than emergency services), from the PACE organization or from an entity authorized by the PACE organization.PACE participants may be fully and personally liable for the costs of unauthorized or out-of-network services.

ELIGIBILITY


To be eligible for PACE, an individual needs to be:

  • 55 or older
  • Certified by the state to need nursing home level of care
  • Able to live in the community with PACE support without jeopardizing your health or safety
  • Live in one of the following zip codes: 70001, 70002, 70003, 70005, 70006, 70032, 70037, 70043, 70053, 70056, 70058, 70062, 70065, 70067, 70072, 70094, 70112, 70113, 70114, 70115, 70116, 70117, 70118, 70119, 70121, 70122, 70123, 70124, 70125, 70126, 70127, 70128, 70129, 70130, 70131

STEPS TO QUALIFY


  1. Initial Qualifcation Over the Phone
  2. Home Visit by the Intake Coordinator
  3. Level of Care Assessment
  4. Collection of Financial Information
  5. Medicaid Application
  6. Visit to the Center
  7. Potential Enrollees are asked to arrive at the center at 9:00am the day of their scheduled visit. They will visit with the PACE staff, and will have an opportunity to interact with current PACE participants. Lunch will not be served. The Interviews include visits with the Doctor and the Social Worker.

 

PLEASE NOTE: PACE services will not begin until the frst day of the month following the enrollment meeting. Potential enrollees with dementia or Alzheimer’s should have a Power of Attorney in place in order to join PACE.
